To request to have your Kentucky voter registration canceled, please complete the following:

Input the PIN number that is written on the top right of the letter you received from the Kentucky State Board of Elections.

Pin Number The Pin Number is required.

Input your birthdate using the following date format: MM/DD/YYYY.

Birthdate The Birthdate is required.
